New NNSW Pres. Sheild Forms

I was just wondering if there was any truth to the rumour that the forms that were handed out for the NNSW presidents sheild team around 2 moths ago can not be used as they don't have a TBA number on them. Does this mean that there is a new set of forms that need filling out and if there are, where could i find them???

Yes the old forms are to be disregarded.
And the new form should be out early next week.
