Here is the entry form and a nomination form for this years Melbourne Cup at Ed Fleming Lanes.
NOTE: The nominations PDF is set up so bowlers can fill in the details direct onto the form then print it out using Acrobat Reader. I hope this makes filling in your nominations easier.
All you have to do is open the PDF then click on the line and start typing. When you have finish filling in the information then print it out ready to post or fax.
If you have a full version of Acrobat then you should be able to save your file ready to email.
